- Session Notes: Notes taken during a session are not kept. Instead, we create a brief report containing only essential details to ensure continuity of care. This is done at the end of each session, allowing a maximum of 24 hours between the end of the session and the destruction of the notes. Until destruction, notes will be kept in a locked place for safety.
Data Retention
Your data is retained for seven years from the date of your last session or until the closure of our practice, whichever occurs first. After this period, all personal information will be securely deleted.
Exceptions to Confidentiality
While we are committed to safeguarding your privacy, there are rare circumstances where we may need to disclose information, including:
- Legal Obligations: If required by law or a court order.
- Risk of Harm: If we believe there is an immediate risk of harm to you or others.
In such cases, we will always aim to discuss the matter with you first (where possible).
Sharing Sensitive Information
We always encourage you to consult a licensed medical professional before booking an appointment. Florence Tyloo is not responsible for any matter related to diagnoses or medical treatment. The services provided at Dementia Partnered Care are no substitute for medical consultation, diagnosis and treatment.
During our sessions, you may choose to share sensitive or personal information, including health-related details. While this is entirely voluntary and may enhance the effectiveness of our work together, you are under no obligation to disclose anything you feel uncomfortable sharing. We respect your right to withhold any information at your discretion.
If you choose to share sensitive data, please know that we handle it with the utmost care, in line with UK GDPR requirements.
